Pay Rate: $30.49 per hour (Paid Biweekly) Location: Remote – North Carolina (Region 2) | Candidates must reside in North Carolina. Schedule: Monday – Friday | 8:00 AM – 5:00 PM EST (40 hours per week) Assignment Dates: July 30, 2026 – December 23, 2026 Assignment Type: Temporary assignment with the opportunity for extension or conversion to a permanent position based on business needs and performance. Position Overview: The 171442 Remote Case Management Coordinator is responsible for coordinating comprehensive care management services for members receiving Long-Term Services and Supports (LTSS). This role focuses on improving member outcomes through care planning, coordination of healthcare services, education, and collaboration with providers, caregivers, and community resources. While this is a remote position, regular face-to-face member visits within the assigned territory are required. Responsibilities: Evaluate members' medical, behavioral, and social service needs. Develop and maintain individualized long-term service and care plans. Coordinate care between members, caregivers, physicians, specialists, providers, and community organizations. Monitor member progress and update care plans based on changing needs. Educate members and families on available healthcare benefits, community resources, and long-term care services. Assist members in accessing housing, transportation, employment, independent living resources, and other support services. Conduct member assessments through home visits and other approved locations. Collaborate with interdisciplinary care teams to ensure quality, cost-effective care. Maintain accurate documentation and case records in compliance with state, federal, and organizational guidelines. Identify barriers to care and recommend appropriate solutions. Participate in quality improvement initiatives and provide feedback to leadership. Perform additional duties as assigned. Work Environment: Remote position supporting members throughout North Carolina Region 2. Face-to-face home visits are required. Travel may exceed 50% depending on member needs. Occasional in-person team meetings. Standard Monday–Friday work schedule with no overtime required. Assigned Coverage Area: Candidates should be comfortable traveling throughout the following North Carolina counties: Alleghany Ashe Davidson Davie Forsyth Guilford Randolph Rockingham Stokes Surry Watauga Wilkes Yadkin Required Qualifications: Bachelor's degree. Active Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW) license. Minimum of two (2) years of LTSS and/or HCBS care coordination or care management experience. Experience with care delivery monitoring and service coordination. Background in social work, geriatrics, gerontology, pediatrics, human services, or related healthcare fields. Strong assessment, communication, documentation, and organizational skills. Proficiency with computer systems and electronic documentation. Ability to manage multiple cases while working independently. Reliable transportation and ability to travel throughout the assigned territory. Preferred Qualifications: Managed care experience. Medicaid population experience. Knowledge of Long-Term Services and Supports (LTSS). Knowledge of Home and Community-Based Services (HCBS). Experience connecting members with community resources.
