They will collaborates with a multidisciplinary staff to meet Population Health and quality improvement goals through projects aligned with the mission of Apicha and its strategic goals. The Population Health Manager supports the implementation of strategies to drive clinical transformation toward value-based care delivery and addressing underlying health disparities and social determinants of health. The role encompasses analysis and review of trends and projections of key clinical metrics, supporting new program development, and making recommendations to ensure the viability of the organization over the short and long term.
