Key Responsibilities
- Engage proactively with clients to identify early signs of distress and implement preventative strategies to divert them from crisis services.
- Maintain up-to-date crisis alerts to coordinate effectively with county crisis services and ensure timely interventions when necessary.
- Facilitate seamless transitions for clients moving to and from hospital and inpatient services, collaborating closely with healthcare staff to support discharge planning.
- Deliver high-quality teletherapy sessions, utilizing evidence-based practices tailored to meet the individual needs of a varying patient population.
- Document clients' progress and treatment plans in compliance with state regulations and clinic policies, ensuring continuity of care.
- Participate in clinical meetings to discuss patient cases and share insights to enhance overall treatment strategies within the team.
